How long should these be heated for in a 1000 watt microwave?

It depends how hot you want it but we heat it up for 1 minute on High, flip it over and heated up an additional minute on high

Does the microwave have to have a carousel/turntable?

No, I wouldn't think so. A turntable just aids more even heating of anything.

Do I have to add water to it before I warm it?

No water!! It is filled with different scents and maybe rice or beans??? You put it in the microwave for about 2 minutes and then put it on your neck and shoulders…it is wonderful!!!
