Does this cable also convert from balanced to unbalanced without modifying the mic cable? Or do you have to ground the shield on the mic cable?

if you are going into a mic with an XLR connector on it you should be OK, I modified a Kenwood mic I had so I'm going from Kenwood 8 pin through this cable which is XLR back to a 8 pin so I had to solder my mic cable anyway if that makes sense

This cable has the 1/4 inch phono plug for the foot switch, am I correct?

IS this a female xlr on this cable?

yes, plugs directly into a standard (professional) microphone. I use a Shure SM58 on boom arm into my kenwood ts-2000.
