Is this the vented scleral cup inserter? or the non-vented one?

Non vented but you can cut the bottom off

Anyone know where to buy cheaper knockoffs? 4$ for a piece of rubber is quite a ripoff.

The DMV scleral cups are patented. Anything else would be an illegal product.

Does anyone have a suggestion for a storage case for this item?

There are some that come with cases such as https://smile.amazon.com/DMV-Ultra-Hard-Contact-Remover/dp/B00OJWNY1C/ref=rtpb_1/147-7879238-1305136?_encoding=UTF8&pd_rd_i=B00OJWNY1C&pd_rd_r=59056b20-1942-4a78-99c2-0c310faa721c&pd_rd_w=AGCBV&pd_rd_wg=EyPML&pf_rd_p=e3464036-7ac0-415f-8736-4a31ff917eb7&pf_rd_r=QS1W9X5YPKTGZ45CS94K&psc=1&refRID=QS1W9X5YPKTGZ45CS94K If these plungers are not what you need you can most likely still use the cases from your other ones. Do a search, as there are some that come with a nice carry case as well.

Would this tool work for inserting OrthoK lenses (which are much smaller in diameter?

I use mine to take out my Schleral lens as it is like a suction cup. Not sure how it would work for insertion. Ask in the eye dept at any of the chain stores if you don't get an answer here.
