Does this slider have a two-tripod mounting option, or can you only mount it to a single tripod?

Yes it has tripod mounting options at both ends and on the very end for use upright. I hope that answers your question for you. It's a good heavy well made slider and seems to be very smooth.

What size of the screw attached on top of the slider?

Comes with 1/8” camera thread screw and 3/8” bushing to which I mount a heavy duty ball head. You can screw your camera to the rail slide but a ball head allows you to tilt the camera.

What is the width of the slider track?

The slider rail is 1 1/2 inches in width, and the base where the camera goes on is 2 1/4 inches, the length is 3 inches. I hope this helps.

I am curious how the slide track mounts to the tripod. I have a ProVista 7518 Tripod. Will it mount to the shoe plate?

It's a 1/4" mount, which is the size most video tripod quick release plates have. But the thing is too long for a single tripod. It's going to shake like crazy anywhere but the middle. It has 1/4" mounts on either end also. So your best bet is to use two tripods, or use light stands that have a 1/4" mount instead of a stud.
